Percussion Therapy

A handheld device that delivers rapid, concentrated pulses of pressure into muscle and fascia — easing tension, driving circulation, and keeping tissue pliable between visits. Used correctly, it is one of the most useful recovery tools a patient can own. Used carelessly, it is one of the easiest to misuse. This page is how we use it.


Important — Float It, Don’t Force It

The most common percussion-therapy mistake is pressing hard and grinding into a sore spot. You do not need force — the percussion does the work. Rest the device on the muscle, glide slowly, and keep it on the soft muscle belly. Percussion is not appropriate over bone, joints, the spine, the front of the neck, or a fresh injury. If you take a blood thinner, have a bleeding or clotting disorder, osteoporosis, a recent injury or surgery, or are pregnant, talk with your physician before using a percussion device. The guidance below is what we share with our patients after a clinical evaluation.


Section 1

What Percussion Therapy Actually Is

Percussive therapy — often called a “massage gun” — uses a motorized head to strike the soft tissue dozens of times per second, combining rapid percussion with vibration. Each pulse moves the muscle and the fascia around it through a small, fast range, producing effects that a static stretch or a thumb simply cannot reproduce at that speed.

At The Fascia Institute we think of percussion as a self-applied soft-tissue tool — a way for you to keep muscle and fascia mobile and comfortable at home, between the work we do in clinic. It is not a treatment for a specific diagnosis on its own, and it is not a substitute for Hydrofascia Release™ or your prescribed rehabilitation. It is a maintenance layer: the thing that keeps tissue pliable so the rest of the plan holds.


Section 2 · How It Works

Four effects working together.

Percussion is not a passive buzz. The rapid mechanical input produces measurable changes in circulation, pain signaling, and tissue behavior — which is why it earns a place in a recovery plan.

Effect 01 · Circulation

Blood flow & fluid movement.

Rapid percussion drives local blood flow and lymphatic drainage — delivering oxygen to worked tissue and helping clear the metabolic by-products that accumulate after activity. More flow is the foundation most of the other benefits build on.

Effect 02 · Pain

Pain modulation (gate control).

High-frequency vibration floods the nervous system with sensory input that competes with and dampens pain signals — the gate-control mechanism. The practical result is a tight, guarded area that simply feels less sensitive and is easier to move.

Effect 03 · Fascia

Fascial glide & pliability.

Mechanical input helps mobilize and hydrate fascia and the sliding layers between muscles — restoring the slide-and-glide that stiff, bound, or chronically loaded tissue loses. This is the effect most relevant to the work we do in clinic.

Effect 04 · Tone

Muscle tone & warm-up.

Percussion lowers resting muscle tension and primes muscle for work. A short pre-activity pass improves short-term range of motion and how the muscle fires — without the aggressive static stretching that can leave hypermobile tissue more lax, not less.


Section 3

Two Jobs: Prime Before, Recover After

Use 01 · Prime

Warm up tissue before you move.

A brief pass on a moderate speed before training, physical therapy, or a long day raises local blood flow and primes muscle — improving short-term range of motion and readiness. Think 30–60 seconds per muscle group, not a full session.

Use 02 · Recover

Flush soreness after you move.

Slower, longer passes after activity ease delayed-onset muscle soreness, soften tight muscle and fascia, and support recovery — and help maintain the pliability we create during a release session, so the gains last longer between visits.


Section 4

How To Use It — The Four Rules

1. Start low and float. Begin on the lowest speed and rest the device on the muscle — let its own weight set the pressure. Ramp the speed up only as the area stays comfortable. You should never have to push.

2. Glide slowly, with the grain. Move along the length of the muscle about one inch per second — a slow, deliberate pass, not fast back-and-forth scrubbing. Follow the direction the muscle fibers run.

3. Stay on the muscle belly. Keep the head on soft muscle — never on bone, joints, the spine, or bony prominences. Keep it moving; do not hold it on any single point for more than a few seconds.

4. Keep sessions short. About one to two minutes per muscle group, and only a few areas per session. With percussion, more is not better — and a sore, bruised muscle the next day means you used too much pressure or time.


Section 5 · Safety

Where Not To Use It

Do not use a percussion device over:

  • Bone, the spine, joints, or bony prominences
  • The front or sides of the neck, or the throat
  • A fresh injury, sprain, strain, fracture, or recent surgical site
  • Open wounds, rashes, skin infection, or any area of numbness
  • Known blood clots, or directly over major arteries
  • The abdomen, the kidneys, or directly over internal organs

Talk with us or your physician before use if you take a blood thinner, have a bleeding or clotting disorder, osteoporosis or fragile bone, a tumor, a nerve condition, or a pacemaker, or if you are pregnant — in which case avoid the abdomen and low back entirely. Stop immediately and contact us if you notice sharp pain, numbness, tingling, bruising, or pain that increases rather than eases.


A Note For Our Hypermobility & EDS Patients

If you live with hypermobility, EDS, or HSD, percussion can be genuinely helpful for muscle tension and guarding — but the rules matter even more. Stay on a low speed, work the muscle away from loose or unstable joints, and never use percussion to push a joint further into its range. Your joints already move too much; the goal is to calm the muscle that is working overtime to stabilize them, not to loosen the area further. Use the device only on the regions, settings, and timing we mapped out together at your visit.
