FIT Los Angeles Financial Policy:

FIT LA operates as a fee-for-service practice and does not participate in or bill any insurance plans (including Medicare, Medi-Cal, or private insurance).

Payment for all services is due at booking.

All payments are non-refundable.

FIT LA does not submit insurance claims, prior authorizations, or medical-necessity forms.

Patients may request a superbill to seek out-of-network reimbursement directly with their insurance carrier.
Under the No Surprises Act, self-pay and uninsured patients are entitled to a Good Faith Estimate (GFE) upon request or when scheduling.
Rescheduling & Missed Appointments
Please provide 48 hours’ notice for any changes. All prepaid appointments are non-refundable. With timely notice or an approved reason, credit may be applied to a future appointment. After two reschedules, any remaining credit becomes a non-refundable cancellation fee.
