What is Diastasis Recti Abdominis (DRA) and Its Symptoms?

Diastasis recti abdominis (DRA) is a condition where the rectus abdominis muscles, commonly referred to as the “six-pack muscles,” separate due to the weakening of the linea alba—a collagen-rich fascial structure. This separation is typically caused by increased abdominal pressure, which weakens the connective tissue.

Studies commonly define a separation greater than 2 cm at 3 cm above the umbilicus as indicative of DRA, though measurement methods may vary. Symptoms of DRA include abdominal discomfort, pain, bloating, and pelvic floor dysfunction. In severe cases, it can lead to hernias, significantly affecting quality of life. Dysautonomia, a condition involving dysfunction of the autonomic nervous system, has also been associated with DRA in some cases, potentially exacerbating symptoms like fatigue and dizziness.

While researchers continue to explore the precise causes and risk factors of DRA, pregnancy and high BMI are widely recognized as key contributors.

Common Risk Factor: Pregnancy

Pregnancy is a leading risk factor for DRA, as it causes a temporary increase in intra-abdominal pressure, stretching the fascia and weakening abdominal muscles. As the uterus expands during pregnancy, the connective tissue of the abdominal wall thins, and the rectus abdominis muscles separate. Hormonal changes, including the release of relaxin, further contribute to the elongation of the fascial structures.

Although the fascia and muscles often recover postpartum, the separation may persist in many cases. Women who have had multiple pregnancies are at an increased risk of developing DRA due to the repeated stress on the abdominal wall.

Additionally, women who have undergone two or more cesarean sections (C-sections) may face a higher likelihood of DRA compared to those who have had only vaginal deliveries.

Common Risk Factor: High BMI

High BMI is another significant risk factor for DRA. Unlike pregnancy, which temporarily increases intra-abdominal pressure, obesity exposes the abdomen to chronic pressure. This prolonged strain weakens the linea alba and separates the rectus abdominis muscles.

Obesity is often accompanied by reduced muscle mass, which further compromises the integrity of the abdominal wall, making DRA more likely.
